Before you get started you will need the following information to complete the process:

• A signed W-9 form providing the Federal Employer Identification Number (FEIN).

 

If you would like to be considered for work over $300,000, the documents listed below are required:

• Most recent 3 years of Workers Compensation Experience Modification Rates.

• Signed letter from your Surety Agent, along with the related power of attorney, stating the Surety name, your Company’s per project and aggregate limits. This letter must be dated within the last 12 months.

• If your company has more than 10 employees, most recent 3 years’ OSHA 300 and 300A. Due to the personal information on these logs, please black out the employees’ names prior to submittal.

• Most recent 3 years’ audited financial statements.

 

Once you have completed the pre-qualification, you will be notified via email the results of our review. Subsequently, you will be contacted annually to provide updated information.
